Whiplash is the lay term for mild traumatic injuries to the neck.  Sensitive tendons, muscles, and ligaments work together in your neck.   A strain of the neck is when the muscle or tendon is suddenly contracted or stretched.  A sprain is when the ligaments are stretched or torn. It is very difficult to diagnose a neck problem with any certainty without Magnetic Resonance Imaging (MRI) films.  With an MRI, a doctor can determine the extent of your neck problem and recommend the best treatment.

The neck (cervical spine) is made up of discs with a hard shell and a soft gel filled interior.  This works to absorb force and protects the spine. When the gel inside the disc protrudes, due to trauma or degeneration, this is known as a herniated disc.  Symptoms include a shooting pain that radiates down the arms, numbness or tingling and muscle weakness.

A herniated disc is usually diagnosed with an MRI and clinical testing of reflexes, range of motion and pain level.  Treatment is usually a combination of physical therapy, chiropractic treatment, pain relievers and muscle relaxants. Rarely, surgery is necessary to relieve the pain.

The Medial Collateral Ligament (MCL) is a ligament on the inner part of the knee which prevents over extension.  This type of injury is usually caused by a direct blow to the outside of the knee. Symptoms of a torn MCL include a “popping sound” when the injury occurs, swelling, tenderness in the back of the knee and instability.  MCL do not usually require surgery, but a brace may be necessary for some time due to the instability. Physical therapy is usually very helpful.

ACL Knee Injuries

The Anterior Cruciate Ligament is the most common serious injury in the knee.  The ACL stabilizes the shin bone to ensure that it moves normally. There types of trauma that can cause ACL injuries including:

  • Changing direction quickly
  • Suddenly decreasing speed when running
  • Landing a jump
  • A collision
  • Twisting (when falling)

Symptoms of an ACL tear include a “popping sound” at the time of injury, swelling, non weight bearing and instability.  Doctors usually use an MRI and clinical correlation to diagnose a torn ACL. If the ACL is sprained and not torn, physical therapy may all that is needed.  For simple torn ACL’s arthroscopic surgery is sometimes recommended. More severe tears may require a surgical ACL reconstruction.

Injuries From the Seat Belt

Seatbelts, while protecting you by preventing you from hitting the windshield or being ejected from a vehicle, can sometimes cause problems of their own.  In a strong collision, they can put significant pressure on your chest, causing bruising. Degrees of Burns

  • First-Degree Burns – These are minor burns where only the outer layer of skin is impacted.  They can be painful, but usually heal completely in a few days. Sunburn, without blistering, is a first degree burn.
  • Second Degree Burns – There are two types of second degree burns.  
    • Partial thickness burns usually look red and may have blisters.  There is damage to both the outer layer of skin (epidermis) and most of the dermis underneath.  Healing usually takes a couple of weeks.
    • Full thickness burns may be white or red and look dry.  There could be a loss of sensation in the affected area.  This type of burn may require a skin graft.
  • Third Degree Burns – This is a very serious type of burn because every layer of the skin is impacted.  They can be black or white, and can look “leathery.” The nerves of the skin are so badly damaged that there may be no sensation at the site of the burn.  Skin grafts will be required and there will be severe scarring.
  • Fourth Degree Burns – These are burns that destroy not just skin, but the muscles and bone underneath.  These burns are often fatal, and survivors need to undergo substantial skin grafting and other life-saving treatments.
  • Fifth and Sixth Degree Burns — These burns cause massive destruction to skin, muscle, ligaments, tendons and bones.  They are almost always fatal.

After the shock of an accident, you are glad to be alive and don’t notice more subtle symptoms of injuries.  Sometimes the symptoms don’t arise until the next day or even several days or weeks later. That’s why it’s important to note any impacts to your body and go immediately to the Emergency Room to report these impacts to your body.  If you send away the ambulance and go home, the police will likely mark down on their report that you were “not injured,” making it difficult to claim these injuries later.

This is not to say that you should “make up” injuries, only that you should report that you “hit your head on the steering wheel” or “hit your side on the door of the car.”  Just report what actually happened in case symptoms arise later. It’s also good to check these parts out as there could even be a minor fracture that requires immediate attention that you are not aware of.  It’s difficult to be sure what you’ve injured and how severely amidst the shock and distractions of an accident.

Headaches – If you’ve hit your head, headaches might commence several days after an accident.  You might have sustained a concussion and not realized it because it didn’t hurt very much at the time and you thought it would just “go away.”  Sometimes these headaches could indicate a more serious problem such as a blood clot or stem from an injury to the neck.

Neck Pain – A whiplash injury is common from a rear end collision and may not seem serious at first.  The next day, or even several days later, stiffness and difficulty moving your head can arise.  These injuries can be diagnosed with MRI’s to understand their severity.   1. Shoulder pain – Orthopedic injuries often do not seem severe until swelling occurs overnight.  Shoulder pain can sometimes be radiating pain from a neck injury, but it could also be caused by a torn rotator cuff or another injury to the shoulder itself.  Medical follow-up is crucial.
  1. Back pain – Back pain after an accident may seem minor until the next day, and worsen if you work at a physical job such as carpentry, home health care or driving a taxi.  Without treatment, back injuries can worsen over time.
  1. Knee Pain – Relieved to be able to get up and walk away from an accident, we may not realize that we have injured our knees until they swell up the next morning.  Knee pain can also be caused by other injuries that alter our gait.
  1. Hand or Wrist Pain – These injuries are common with falls and pedestrian accidents.  Instinctively, we use our hands to break a fall and protect our face.  Relived that the fall has been broken, we walk away and may not notice pain in our hands and wrists until days later.
  1. Abdominal pain or swelling – These are serious symptoms that could indicate internal damage to organs and internal bleeding.  Also looks out for deep purple bruises, dizziness and fainting. Internal bleeding may not have obvious symptoms for hours or days after an accident.  This is a life-threatening situation which requires follow-up at an Emergency Room.
  1. Eye Injuries – Blurry vision due to a head injury could arise quite some time after an accident.  If you notice any vision impairment, you see an ophthalmologist or a neurologist for an evaluation.
  1. Numbness – Loss of feeling or tingling in the shoulders, arms, and hands can be a symptom of a spinal injury or a neurological problem due to a head injury.  Seek medical attention immediately.
  1. Changes in personality or physical function – This could indicate a traumatic brain injury caused by an impact to the head.  Symptoms include fuzzy thinking, memory problem or problems with and hearing. This could also indicate depression.  Medical attention should be sought because these could be symptoms of a traumatic brain injury that could be fatal without treatment.
  1. PTSD – Symptoms of depression and anxiety coupled with vivid and disturbing memories could indicate Post Traumatic Stress Disorder.  Children are more likely to suffer from this than adults, and less likely to want to discuss it. Treatment with a psychologist should be sought.
